A COPD Patient Program

What is STRIVE™ ?
STRIVE™ is an educational program that provides patients, their families, and caregivers living with Chronic Obstructive Pulmonary Disease (COPD) the resources and tools they need to take control of their COPD. 

STRIVE™ will guide you through 6 learning objectives:

 

  • Module 1: Understanding COPD 
  • Module 2: Indoor and Outdoor Triggers and COVID-19 
  • Module 3: COPD, Mental Health, Depression, and Anxiety 
  • Module 4: Diet, Exercise and Energy conservation with COPD 
  • Module 5: COPD and Medication Management 
  • Module 6: Exacerbation and COPD action plan  

 

In partnership with the National Heart, Lung, and Blood Institute’s (NHLBI) Learn More, Breathe Better initiative, participants will receive the following resources and tools: 

 

  • A pedometer to track your activity 
  • A spirometer to test your lung function 
  • Weekly journal booklets to track your thoughts and progress 
  • Direct access to a health educator that can answer your questions 
  • A COPD action plan for you and your primary care physician 
 

In addition, participants of the STRIVE™ program are entered into a research study that allows NHLBI and EFA to learn more about the knowledge gaps about COPD. The study will help to build awareness and understanding of the disease for future generations. 

Do I qualify as a participant of STRIVE™ ?

STRIVE™ is a FREE program and is now available on demand virtually where you can access and complete the learning objectives 

 on your own time and at your own pace. Instructor led in-person workshops are also available upon request. 

Requirements to participate in the program: 

  • Have a physician-diagnoses of COPD
  • Live in Southern California
  • Have access to internet and computer 
  • Actively participate in six 20-40 minute modules 
  • Willingness to complete a pre- and post-survey in each module and two follow up surveys administered over six months by our health educators 
  • Use of the spirometer, pedometer, and journal booklets to document weekly progress

  • COPD is currently the 4th leading cause of death in the U.S. and accounts for nearly 160,000 deaths per year.
  • More than 16 million people have been diagnosed with COPD and it is estimated that millions more have it but do not know it.
